THE ENCLOSED FORM MUST BE COMPLETED AND RETURNED TO THE GRAND LODGE, IMMEDIATELY FOLLOWING YOUR ELECTIONS. DATE OF THE 2007 CONVENTION IS JUNE 21-24. THE RESULT OF YOUR ELECTION IS IMPORTANT. PLEASE COOPERATE BY READING THE INSTRUCTIONS AND STRICTLY ADHERING TO THEM.

The name and address of EACH DELEGATE AND ASSISTANT STATE DELEGATE MUST APPEAR PRINTED ON THE REVERSE SIDE OF THE FORM; DELEGATES in the UPPER HALF, listing the PRESIDENT AS FIRST DELEGATE in all cases and the ASSISTANT DELEGATES in the LOWER HALF.

Therefore, the name of the President must appear on “line 1” of the State Delegate listing, followed by any other elected delegates according to the number of votes received at their election (in space provided before each name). THIS IS VERY IMPORTANT!! If the President is also an elected delegate, the name MUST APPEAR TWICE, ONCE AS DELEGATE NUMBER “ONE” and once again, in the position by number of votes received as Delegate. The name(s) of Assistant Delegates (alternates) MUST APPEAR UNDER “Assistant State Delegates” even if they will be filling the seat of a delegate at the Convention. Remember, THIS IS NOT A DELEGATE ELECTION YEAR.

Do not wait for your installation to take place; lists must be received as early as possible to allow for convention committee assignments and printing of credentials, etc. Each year there are lodges that do not send in their officers’ lists, and as a result, officers and delegates do not receive any information regarding convention assignments, credentials and correspondence.
